5 Easy Steps to Access Your PhilHealth Records Online

Need to get a copy of your MDR or check your PhilHealth contributions? You can now do that online!

Kailangan mo ba ng kopya ng iyong MDR or i-check ang iyong mga hulog sa PhilHealth? Pwede na yang gawin online!

Important Note: You need to be a registered member first before you'll be able to access this Portal. If you're not registered yet, visit the PhilHealth office nearest you or contact them to ask for assistance.

Importante: Dapat ikaw ay rehistradong PhilHealth member na bago mo ma-access ang Portal na ito. Kung hindi ka pa registered, maaari kang pumunta sa pinakamalapit na opisina ng PhilHealth o i-contact sila para humingi ng assistance.

You will now learn how to use the new PhilHealth Member Portal by following these 5 easy steps:

Nasa ibaba ang limang (5) madaling hakbang para magamit mo na ang PhilHealth Member Portal:

 

1. Go to memberinquiry.philhealth.gov.ph/member. Then click Create Account.

1. Pumunta sa memberinquiry.philhealth.gov.ph/member at i-click ang Create Account.

2. Encode all the required information in the Member Portal Account Creation page:

2. I-encode ang lahat ng kailangang impormasyon:

a. PhilHealth Identification Number

b. Name (Last Name & First Name)

c. Date of Birth

d. Sex

e. Email Address - Note: Make sure that the email address you will enter here is working because it will be used for account activation later on.

e. Email Address - siguraduhin na nabubuksan ninyo ang email address na ilalagay ninyo dito.

f. Mobile Number

3. Under Login Information, enter your Preferred Password. The requirements for the password is highlighted on this part. You also need to enter code the CAPTCHA image before clicking Create Account.

3. Sa Login Information, ilagay ang nais mong gawing password. May gabay din dito kung ano ang kilakailangang katangian ng iyong password para sa karagdagang seguridad. Kailangan mo ring i-type sa ibaba ang nababasa mong CAPTCHA code. Pagkatapos, i-click na ang Create Account.

4. Next, it will tell you that a link was sent to your email address.

4. Kung tama ang lahat ng mga sagot mo sa form, ipapadala na ang activation link sa iyong email address.

5. Click the link in your email address to complete creating your PhilHealth Member Portal account.

5. I-click lamang ang link na pinadala sa iyong email address para kumpletuhin ang proseso.

Congratulations! You have now created your PhilHealth Member Portal account.

Ipapa-alam sa iyo na naging matagumpay ang pag-gawa mo ng iyong PhilHealth Member Portal account.

You can now proceed to login to finally access your PhilHealth records, generate and print a copy of your PhilHealth Member Data Record (MDR) and check your member contribution payment history.

Maaari ka nang bumalik para mag-login sa iyong account, gumawa at magprint ng kopya ng iyong MDR at i-check ang mga hulog mo sa PhilHealth.
