PhilHealth Forms

The Philippine Health Insurance Corporation (also PHIC or PhilHealth) is the administrator of the National Health Insurance Program (NHIP) and now one of the lead implementing agencies of universal health coverage in the Philippines.

These are the forms that you'll need when transacting with PhilHealth and their respective uses (click the link to the picture of the form to get an editable copy of the form):

1. PhilHealth Member Registration Form (PMRF)

PMRF is used for the registration of new members or updating of records of existing members.philhealth pmrf

2. Report of Employee-Members (ER2)

An ER2 is the form used by employers to list down their employees both new and old. They will indicate here the position, salary and date of employment of their employees.
